Open a new document 600x400 pixels.
Create a new layer ( press Ctrl+Shift+Alt+N )
Set the foreground color to #364ecc
Set the background color to #12195a
Select Gradient Tool and draw a vertical line from top to bottom.
Now we will create a new document because we will need a pattern.
Create a new document. Use this settings:
Zoom this document to 1600 %
With the pencil tool ( 1 Pixel brush ) create something like me:
The colors are #515151 for the square and #959595 for the middle point.
After you have create the same drawing as mine go to Edit > Define Pattern... Choose a name for this pattern and press Ok
Now let's come back to our initial document.
Create a new Layer ( press Ctrl+Alt+Shift+N )
Select the Paint Bucket Tool.
After that select the pattern we have created a few steps ago
And press one time on your document.
This is the result:
Now press one Add Layer Mask
Select Gradient tool and draw a vertical line from top to bottom.
The next step is to change the Blending mode for this layer to Overlay
This is my result:
Now with your Line Tool draw some horizontal lines ( Be sure you have the Line Weight set to 1 px )
Then start drawing some lines like in the following image ( it will be god if you create first one horizontal line and after that you can duplicate a few times and place it like me )
Now in Layers Palette select all the lines you have created and press Ctrl+E ( this will merge all layers in only one layer )
Now Go to Edit > Transform > Warp
Be sure you hit enter after you choose Flag option.
Now press one more time on Add Layer Mask
Select Gradient tool one more time and draw a vertical line from bottom to top
Now add some text and you are ready.
This technique is very good for banners , signatures or headers...
