1. Opening the document

Open a new document, size 395 px by 116 px.

2. Making the selection

Make a selection as shown. To do this, first make a rectangle using the rectangular marquee tool, then select the circular marquee tool. With this tool, hold keys Shift + Alt, and drag from somewhere around where the arrow starts (in the picture shown below) , to where it finishes. You might have to experiment a little.

3. Making the logo

Duplicate the layer, hold Shift and press the Left Arrow key four times.

4. Repeating

Repeat step 3 on the duplicated layer. You should now have something like this:

5. Merging and adding gradient

Merge the three layers into one. Now apply a Gradient Overlay by right-clicking on the merged layer, and clicking Blending Options, and then selecting Gradient Overlay. Leave it default except for the colours. Use these settings for the colours:

That's the hardest part done already! Now, you just have to add some text and a border.

6. Adding the text

Add some text saying your company name, a little profile about yourself, and maybe your company slogan. I used #8F8F8F for the colour of the text, and fonts MagistralC and 04b_21. Here's what I got:

7. Adding the border

Now we just need a simple border. Make a new layer and name it 'Border'. Go Edit>Stroke and use these settings:

8. Finished!

Click OK and you're done! This is the end result:
